Carpal tunnel syndrome in rheumatoid arthritis patients: the role of combined ultrasonographic and electrophysiological assessment

چکیده
Abstract Background Carpal tunnel syndrome (CTS) is known as one of the most common neurological extra-articular manifestations in rheumatoid arthritis (RA) patients. Studies on CTS RA depend mostly electrophysiological assessment. Few studies have used ultrasonography for evaluation local causes with much focus wrist and tenosynovitis main cause entrapment neuropathy median nerve RA. The aim our study to assess carpal patients by whether inflammatory or anomalous variations could affect decision-making patient management. Results was diagnosed 71 out 74 examined wrists conduction (NCSs) categorized from minimal severe according Padua et al.’s (Ital J Neurol Sci 18:145–50, 1997) grading criteria. Median CSA at level inlet flattening ratio showed statistically significant relation severity. Bifid MN found 20 (10 mild 10 moderate wrists), a persistent artery 4 CTS, an accessory muscle bundle present 3 (2 1 CTS). majority hands (85.1%) flexor tendon radio-carpal joint synovitis. US7-joint score using GSUS7 & PDUS7 synovitis, erosions patients’ disease activity DAS28 score. Significant relations between severity following studies’ parameters, distal motor latency (DML), motor/sensory NCV, peak sensory latency, amplitude SNAP, median-radial difference test, were observed. Conclusion Synovial inflammation compression such bifid nerve, artery, are collectively contributing factors etiology Ultrasonographic visualization these enables early detection highlights possibility non-arthritic-related causes. Using 7-joint ultrasound (US7) assessment tenosynovitis, valuable role reflecting its development

[Carpal tunnel syndrome in rheumatoid arthritis].
The term carpal tunnel syndrome (CTS) refers to the clinical condition presenting with paraesthesiae or dysaesthesiae in the median nerve distribution, and coincidental impairment of function of those muscles of the thenar eminence supplied by the median nerve, with or without objective motor and sensory signs. This was first adequately described by Brain, Wright, and Wilkinson (1947) and is at...
متن کاملThe Usefulness of Calculated Electrophysiological Parameters in Assessing the Severity of Carpal Tunnel Syndrome
Background: Carpal tunnel syndrome (CTS) is the most common type of mononeuropathy. Although CTS patients usually have sensory and motor latency in the EMG-NCV or EDX, a significant percentage (10-25%) of patients have a normal routine study. And if more complete diagnostic tests are not performed, it will not be diagnosed. We decided to identify this group of patients by calculating other meth...
متن کاملEffect of low level laser therapy in rheumatoid arthritis patients with carpal tunnel syndrome
OBJECTIVE: the aim of the present study was to evaluate the efficacy of low level laser therapy (LLLT) in patients with rheumatoid arthritis (RA) with carpal tunnel syndrome (CTS). MATERIAL AND METHODS: a total of 19 patients with the diagnosis of CTS in 19 hands were included and randomly assigned to two treatment groups; LLLT (Group 1) (10 hands) with dosage 1.5 J/ per point and placebo laser...
متن کاملCarpal tunnel syndrome and subsequent rheumatoid arthritis in the 'fibrositis' syndrome.
Eight of 11 patients who satisfy Smythe's criteria for the 'fibrositis' syndrome have subsequently developed carpal tunnel syndrome. Six of the 8 patients have gone on to develop an inflammatory polyarthritis, 3 of whom are seropositive and satisfy the criteria for classical rheumatoid arthritis. The relationship between the 'fibrositis' syndrome and rheumatoid arthritis is not clear, but the a...
